Which 21 panel cup fits your program?
Treatment and clinical programs
Choose a CLIA waived cup. CLIA waived means the FDA has categorized the test as simple to perform. A site that uses waived tests for clinical purposes still needs a CLIA Certificate of Waiver from CMS and must follow the manufacturer's instructions.

Forensic Use Only cups are made for forensic drugs-of-abuse testing, and ours add strips such as Tramadol, Ketamine, Ecstasy (MDMA), Barbiturates and Tianeptine (ZAZA). 3 of the 21 panel cups also check for adulterants (pH, specific gravity and creatinine). Forensic Use Only tests are not for clinical diagnosis, treatment decisions or home use.
3 Forensic Use Only 21 panel cupsWorkplace testing (non-DOT)
Instant cups suit company-policy testing: pre-employment, random, reasonable suspicion and post-accident. They can't be used for DOT-required testing, which must go to an HHS-certified laboratory under 49 CFR Part 40. Tell us about your program and we'll point you to the right configuration.

How to read the result
- Negative
- Two colored lines appear: one in the control region (C) and one in the test region (T) for that drug. A line in T of any intensity, even a faint one, means negative for that drug.
- Preliminary positive
- Only one colored line appears, in the control region (C). No line appears in the test region (T) for that drug. This means the drug may be present at or above the cutoff level.
- Invalid
- No line appears in the control region (C). Discard the test and repeat with a new device. If it happens again, contact us.
Always follow the package insert for the specific test. Preliminary positive results should be confirmed by a laboratory using GC-MS or LC-MS/MS.

Questions about 21 panel drug tests
What does a 21 panel drug test test for?
A 21 panel drug test screens one urine sample for 21 drugs or drug classes at the same time. Almost every 21 panel cup we sell tests for Kratom, Synthetic cannabinoids (K2), Fentanyl, Alcohol (EtG), Amphetamine, Opiates, Methamphetamine, Benzodiazepines, Cocaine, Methadone, Oxycodone, Buprenorphine, Marijuana (THC) and Phencyclidine (PCP). The remaining strips vary by cup: depending on the configuration, they add Tramadol, Ketamine, Ecstasy (MDMA), Barbiturates, Tianeptine (ZAZA), Xylazine, Psilocybin (mushrooms) and Dextromethorphan (DXM). The table on this page lists every drug and cutoff for each cup.
Does a 21 panel drug test test for alcohol?
Only if the cup includes an EtG strip. 3 of the 3 cups on this page do, at 300 ng/mL and 500 ng/mL. EtG is a breakdown product of alcohol that stays in urine longer than alcohol itself.
Does a 21 panel drug test detect fentanyl?
3 of the 3 cups on this page include a fentanyl strip, with cutoffs of 10 ng/mL and 20 ng/mL. A lower cutoff picks up smaller amounts. Standard opiate strips do not detect fentanyl, so you need a cup with a fentanyl strip.
Can a 21 panel cup be used for DOT testing?
No. Under 49 CFR Part 40, DOT-required tests must be collected on a federal custody and control form and analyzed at an HHS-certified laboratory. Our cups are for non-DOT programs, such as company-policy pre-employment, random, reasonable-suspicion and post-accident testing.
Do positive results need to be confirmed?
Yes. A positive result on an instant test is preliminary. Before any legal, employment or clinical action, have the specimen confirmed by a certified laboratory using GC-MS or LC-MS/MS. 12 Panel Now isn't affiliated with a specific laboratory, so customers arrange confirmation testing independently.
What is the difference between CLIA waived and Forensic Use Only?
CLIA waived means the FDA has categorized the test as simple to perform. A site that uses waived tests for clinical purposes still needs a CLIA Certificate of Waiver from CMS. Forensic Use Only tests are for forensic drugs-of-abuse testing by courts, probation and parole, law enforcement, corrections and other forensic programs, as described on the label, and are not for clinical diagnosis, treatment decisions or home use. 0 of our 21 panel cups are CLIA waived and 3 are Forensic Use Only.
How much does a 21 panel drug test cost?
Our 21 panel cups start at $3.99 a cup at the minimum order of 25 and go down to $2.59 a cup at 20,000+ cups. Every cup has a volume price ladder, and shipping is free from 100 cups.
