Test for what is in the supply now

Fentanyl has replaced heroin in much of the supply, and standard opiate strips don't detect it. Add a fentanyl strip, and consider xylazine and nitazenes where they show up locally. Alcohol needs its own test: EtG shows drinking that a breath test misses.

Confirm medication adherence

For clients on buprenorphine or methadone, a cup with BUP or MTD shows whether the medication is present. A missing medication result is a question for the clinician, and it should be confirmed by a laboratory before any decision.

Use a label that fits clinical work

Clinical programs should use CLIA waived tests. A site that uses waived tests for clinical purposes needs a CLIA Certificate of Waiver from CMS and must follow the manufacturer's instructions.

Protect the sample

Check the temperature strip right after collection, and use a cup with adulterant checks (pH, specific gravity and creatinine) when tampering is a concern. Your program policy decides when collections are observed.

How to read the result

Negative
Two colored lines appear: one in the control region (C) and one in the test region (T) for that drug. A line in T of any intensity, even a faint one, means negative for that drug.
Preliminary positive
Only one colored line appears, in the control region (C). No line appears in the test region (T) for that drug. This means the drug may be present at or above the cutoff level.
Invalid
No line appears in the control region (C). Discard the test and repeat with a new device. If it happens again, contact us.

Always follow the package insert for the specific test. Preliminary positive results should be confirmed by a laboratory using GC-MS or LC-MS/MS.

What drugs should a treatment center test for?

Start with the drugs on your clients' history, then add the ones changing the supply: fentanyl, xylazine and, increasingly, nitazenes. Add EtG for alcohol and BUP or MTD for clients on medication-assisted treatment.

Do outpatient programs drug test?

Most do, as part of the treatment plan. How often is set by the program and the clinician, not by the test.

Do positive results need to be confirmed?

Yes. A positive result on an instant test is preliminary.
